Aged Health and
Rehabilitation Program

The Aged Health and Rehabilitation Program

  • Provides services to frail older people living at home, people with disabilities and carers of aged or disabled people.
  • Incorporates the Port Phillip Community Rehabilitation Service which provides a specialised rehabilitation program for residents of the City of Port Phillip.

 

The program is multi-disciplinary and services offered include:

  • Advocacy
  • Care Planning and Co-ordination
  • Carer Support
  • Community Nursing
  • Dietetics
  • Health Promotion and Education
  • Occupational Therapy
  • Physiotherapy
  • Podiatry
  • Social Support and Recreation
  • Social Work
  • Speech Pathology


Inner South East Post Acute Care (ISE PAC) provides services to people after discharge from Caulfield General Medical Centre, the Alfred and Sandringham hospitals during their recuperative period. Care packages are tailored to each individual’s need and may include home care, personal care, nursing, delivered meals, allied health, childcare and transport. ISE PAC services are targeted to people without supports at home and who may be vulnerable to re-admission.

In 2006, our Post Acute Care Service  - part of the Port Phillip Rehabilitation Service - undertook a major client survey, with 78% of respondents reporting that PAC services made a major difference to their recovery.

Additional activities

In addition to these core services, staff are involved in a diverse range of activities.

  • We work closely with hospital services through HARP and the Bayside Health Disease Management clinics, which we established at our St Kilda and SouthPort sites.

  • We ensure access for our hard to reach community members, including different cultural groups, and involve people in the planning of programs. This involves staff participating in many projects such as the Veg Out Gardening Program and the development of health focussed, community building activities and consultations at public housing estates across our area.

  • Through our Older Persons High Rise Support Program we offer a rich range of activities to tenants, including outings, exercise and social groups, community gardens, educational programs and a new program offering social activities for men who generally do not access services. Inclusive communal celebrations such as the Golden Ball and King St Community Day also feature, with support from local agencies.

  • Our Rainbow Place, managed by the Indigenous Steering Group, provides a recognised ‘meeting place’ and point of connection for our Indigenous Community. Many associated activities, and the weekly Wominjika BBQ is well established at Veg Out.

  • All staff work to strengthen partnerships with local agencies as a key way to improve  services for people and also to deliver specific programs such as Diabetes Education, Strength Training and Falls Prevention in 2006.
