Become a member

When you become a member of ISCHS, you will:

• Receive regular information about our services, programs and events and a copy of our annual report
• Be eligible to vote in our Board of Directors elections and/or nominate to be a candidate
• Have the opportunity to participate in our service planning and decision making e.g. attend focus groups, fill out surveys, represent the community on committees
• Have an increased sense of connectedness with community
• Receive invitations to our quarterly health forums
• Receive an invitation to the Annual General Meeting where there will be opportunities to provide feedback
• Be invited to participate in specific focus and advisory groups
  
  

Eligibility

Anyone who is 18 or over and who lives, works or studies in our catchment area or is a carer of an eligible member who is a client of the service is eligible to join as a member. 

Applicants must support the objects of the service and comply with the constitution and the regulations of the company including the commitment to contribute $1 to the company's property in the unlikely event that ISCHS is wound up.

Become a member

Simply fill out a membership form at any one of our centres, call to receive your form in the mail or download the membership form and send it back to us. Your application will be reviewed and endorsed by the Board of Directors at their next monthly meeting. Once approved, we will send out a welcome pack and add you to the ISCHS membership list.
