The Care Plan is here!

What is a care plan?

A Care Plan is a document that outlines your goals.  It is developed together with you and your worker.  

A Care Plan includes:

  • Services involved in your care
  • Goals you want to work toward. For example, these can range from losing weight or stopping smoking, to going out more or going back to work
  • Actions that will help achieve these goals
  • Other partners in your care (such as GP’s, housing services)
  • Reviewing progress
 
Everyone’s service needs are different. Your Care Plan supports your needs, wishes, values and circumstances.

How do we do it?

  • You can ask for a Care Plan or your worker will offer you one
  • Preparing the Care Plan may take one or more visits
  • Usually your Care Plan will be printed for you to keep
  • We will provide you with a folder for your Care Plan

What are the benefits of the Care Plan?

  • You are actively involved in your care
  • Allow us to connect you to services you need
  • Good communication between us
  • You can talk about anything that might be worrying you. For example, some people might want to talk about how they feel lonely or anxious.
  • Identify barriers that stop you achieving your goals
  • We can monitor your progress together
  • Focus on improving and maintaining health as well as dealing with problems as they arise
  • Supports you to manage your long term care

When should we work together on a Care Plan?

At any time in your care process.

Is your information secure?

  • We protect your privacy and confidentiality and comply with privacy laws.
  • If you consent a copy of your Care Plan can be given to outside providers involved in your care.
